1.0 INTRODUCTION
The Federal School of Medical Laboratory Technology (Science), Jos intends to undertake the procurement of various goods and services under the 2021Capital Budgets.

In compliance with the Public Procurement Act 2007, the School invites interested and reputable Contractors with relevant experiences and good tract records to submit documents necessary for consideration for the execution of projects under the 2021 Capital Budget

2.0 SCOPE OF WORK/SUPPLY

Lot 1 FSMLT/LT1/2021 – Procurement & Installation of Solar Grid Systems for Street Lighting
Lot 2 FSMLT/LT2/2021 – Modern (Updating) Classrooms with Virtual Technology
Lot 3 FSMLT/LT3/2021 – Purchase of Teaching/Learning and Equipment for Teaching Research and Diagnostic Purposes
Lot 4 FSMLT/LT4/2021 – Purchase of Reagent/Chemicals/Fittings for Teaching, Research and Diagnostics Purposes
Lot 5 FSMLT/LT5/2021 – Provision of 1 Nos. 1000 Seater Ultra Modern Multi-purposed Auditorium/Equipping of Lecture Halls
Lot 6 FSMLT/LT6/2021 – Provision of Infrastructure/Rehabilitation of Male and Female Students’ Hostels at Permanent Site, Lamingo & Dogon Karfe, Jos
Lot 7 FSMLT/LT7/2021- Equipping (Updating) of General Medical Multi-Purpose Molecular Diagnostic/Research/Reference Centre
Lot 8 FSMLT/LT8/2021 – Partial Completion of New administrative Blocks to serve as Office for Academic Staff at the Permanent Site, Lamingo, Jos
Lot 9 FSMLT/LT9/2021 – Partial Completion of New Administrative Blocks to serve as Office for Non-Academic Staff (Administrative Offices) at the Permanent Site, Lamingo, Jos
Lot 10 FSMLT/LT10/2021 – Procurement and Installation of CCTV for Security Surveillance
Lot 11 FSMLT/LT11/2021 – Rehabilitation, Construction of Water Reticulation and Incinerator at the Male and Female Hostels, Dogon Karfe, Jos
Lot 12 FSMLT/LT12/2021 – Construction/Provision of Road Network for Nupkis Host Community (Social Corporate Responsibility)
Lot 13 FSMLT/LT13/2021 – Rehabilitation and Furnishing of some Selected Primary and Secondary School destroyed by bandits in Plateau North Senatorial District

3.0 ELIGIBILITY REQUIREMENTS
Interested and competent contractors, Suppliers and service providers/consultants wishing to carry out any of the projects above are required to submit the following documents, which will be subjected to verification and due diligence:
Mandatory/Responsive Requirements: Failure to fulfill any of the under-listed requirements will render a bid, disqualified.
(i) Evidence of Certificate of Incorporation with Corporate Affairs Commission (CAC) including Forms CAC2 and CAC7
(ii) Evidence of Company’s Income Tax Clearance Certificate for the last three (3) years valid till 31 December, 2020.
(iii) Evidence of current Pension Compliance Certificate valid till 31 December, 2020.
(iv) Evidence of current Nigerian Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2020.
(v) Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31 December, 2020
(vi) Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring 31/12/2020 or valid certification issued by BPP.
(vii) Sworn Affidavit disclosing whether or not any officer of the relevant committees of the Federal School of Medical Laboratory Technology (Science), Jos or the Bureau of Public Procurement is a former or present Director, Shareholder or has any pecuniary interest in the bidder and to confirm all information presented in its bid are true and correct in all particulars.
(viii) Company’s Audited Accounts for the last three (3) years (2017, 2018, 2019)
(ix) Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed.
(x) Company’s profile with Curriculum Vitae of key staff to be deployed for the project, including copies of their Academic/ Profession qualifications.
(xi) Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, valuation Certificates, Job Completion Certificates and Photographs of the projects.
(xii) Works: List of Equipment with proof of Ownership/Lease
(xiii) For Supply of Equipment: Letter of Authorization as representative of the Original Equipment Manufacturers (OEMs)
(xiv) For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner);
(xv) All documents for submission MUST be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (IRC) as issued by Corporate Affairs Commission (CAC), Contact Address, Telephone Number and e-mail address. The Letterhead Paper MUST bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the company/firm.

4.0 COLLECTION OF TENDER DOCUMENTS
Interested companies/firms are to collect the Standard Bidding Documents (SBD) from the Accounts Department of the School on evidence of payment of a non-refundable tender fee of N10, 000.00 per Lot, paid into the Federal School of Medical Laboratory (Science) Jos Remita account in any commercial bank in Nigeria from 9am -3pm working days, (Remita Payment procedures can be obtained from the Accounts Department of the School).

5.0 SUBMISSION OF TENDER DOCUMENTS
Prospective bidders are to submit bid for each of the Lot desired, two (2) hard copies each of the Technical and Financial bids with soft copy of financial bid only in MS Excel format, packaged separately in sealed envelopes and clearly marked “Technical Bid” or “Financial Bid”. Thereafter, put the two sealed envelopes together in bigger sealed envelope address to THE PROVOST/CEO, FEDERAL SCHOOL OF MEDICAL LABORATORY TECHNOLOGY (SCIENCE) JOS, PLATEAU STATE, and clearly marked with THE NAME OF THE PROJECT AND LOT NUMBER. Furthermore, the reverse of each sealed envelope should bear the name and address of the bidder and drop in the designated Tender Box at the Procurement Department on or before 12:00 noon on Monday, 15 March, 2021. ii. Tax clearance Certificate for the past three (3) years;
iii. Audited account for the past three (3) years;
iv. Evidence of competence/experience;
v. Compliance with extant regulations that have to do with:
a. Employee’s Compensation Scheme (ECS)
b. PENCOM Scheme
c. Industrial Training Funds (ITF) etc;
