1.  INTRODUCTION
The National Pension Commission (PenCom) in compliance with the Public Procurement Act, 2007, is hereby inviting reputable, experienced and interested consultants to submit Expression of Interest (EOI) for the provision of the under-listed services.

2.  SCOPE OF SERVICES: CONSULTANCY SERVICES

3.  ELIGIBILITY REQUIREMENTS
Interested firms should submit two (2) bound EoI documents duly signed, paginated, separated by dividers and arranged in the order outlined hereunder:
(a)      Evidence of Certificate of Incorporation issued by Corporate Affairs Commission (CAC) including Forms CAC2 and CAC7, or Business Name where applicable;
(b)     Evidence of Firm’s Tax Clearance Certificate (or Personal Income Taxes of the Partners where applicable) for the last three (3) years valid till 31st December, 2020;
(c)      Evidence of current Pension Compliance Certificate valid till 31st December, 2020;
(d)     Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2020;
(e)      Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2020;
(f)      Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2020 or valid Certificate issued by BPP.
(g)     Evidence of Firm’s registration with relevant professional regulatory body(ies);
(h)     A Sworn Affidavit:
ï‚·Disclosing whether or not any officer of the relevant committees of National Pension Commission or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
ï‚·That the Company is not in receivership, subject to any form of insolvency or bankruptcy proceedings or the subject of any form of winding up petition or proceedings; and
ï‚·That the Company does not have any Director who has been convicted in any country for criminal offences relating to fraud or financial impropriety or criminal misrepresentation or falsification of facts relating to any matter;
(i)      Company’s Audited Accounts for the last three (3) years – 2017, 2018, 2019;
(j)      Evidence of financial capability to execute the project by submission of Reference Letter from a reputable Commercial Bank in Nigeria indicating willingness to provide credit facility for the execution of the project when needed;
(k)     Company’s Profile with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications;
(l)      Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects;
(m)    Evidence of registration as a Data Protection Compliance Organisation issued by National Information Technology Development Agency for Lot 1;
(n)     Lot 6: Evidence of current registration with Financial Reporting Council of Nigeria (FRCN);
(o)     For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner);
(p)     All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorised officer of the firm

EXPRESSION OF INTEREST FOR THE DEVELOPMENT OF OPEN SOURCE SOLUTION FOR ELECTRONIC MEDICAL RECORD AND INVITATION FOR BIDS (IFB) FOR THE SUPPLY OF ICT HARDWARE AND MEDICAL COMMODITIES.

1.BACKGROUND
The Lagos State Ministry of Health (LSMoh) is a Principal Recipient (PR) for Integrated Global Fund Grant comprising of HIV/AIDS, Tuberculosis and Resilient Sustainable Systems for Health (RSSH). As the Principal Recipient, the Ministry of Health is legally responsible for the management of the integrated grant ensuring achievement of the programmatic results and accountability of the grant financial resources. To fulfill the mandate of the Principal Recipient, the Ministry of Health has established the Grant Management Unit (GMU) to carry out the functions of the PR. The Grant Management Unit works with other implementing units (TB, HIV, RSSH) within the LSMOH, Sub- Recipients (SRs) and other implementing partners in executing its mandate and ensuring that the grant is managed and implemented in compliance with grant agreement signed between the Global Fund and the Principal Recipient.

2.This National Competitive Bidding (NCB) process as specified by the State Procurement Guidelines is open to all bidders in the country as defined in the Guidelines-www.lagosppa.gov.ng.

3.SCOPE OF WORK
3.1.SCOPE OF WORK FOR DEVELOPMENT OF OPEN SOURCE SOLUTION FOR ELECTRONIC MEDICAL RECORD BASED ON OPENMRS
The chosen firm is expected to undertake the following tasks to Implement this pilot project at 10 hospitals with scaling up to additional sites later.
ï‚·Requirements gathering & process re-engineering: This will involve the elicitation and assessment of current business processes and workflows of the designated facilities.
ï‚·Deployment/Installation: This would generally involve deploying an instance of the EMR on a secured dedicated cloud hosting platform or a physical server and connect the system to the DHIS2 used on the national level.
ï‚·Security: The solution should be deployed in Ubuntu operating system. It should be accessible over a public IP and a domain name and secured with an SSL certificate.
ï‚·UAT and Training: This entails pseudo/live data population, user profile creation, end-user training and interaction with the EMR.
ï‚·Go-live: Upon successful UAT, the next step would entail moving to a live/production environment. A back-up would be done, and data migrated on to the live environment. . Maintenance and support: These entail on-going support for the duration of the EMR life cycle.

4.GENERAL QUALIFICATION REQUIREMENTS FROM INTERESTED PARTIES
ï‚·Evidence of Registration with Corporate Affairs Commission (CAC) with inclusion of Certificate of Incorporation and Articles of Association.
ï‚·Company profile and organizational structure with full name of organization and contact person, postal address, telephone/fax numbers, and e-mail addresses.
ï‚·Evidence of company tax clearance for the last 3 years (2017-2019)
