b. Evidence of Company’s Incomes Tax Clearance Certificate for the last three (3) years (2017,2018 & 2019) valid till 31st December, 2020; with minimum average turnover of N20 million;
c. Evidence of current Pension Compliance Certificate valid till 31st December, 2020;
d. Evidence of current Industrial Training Fund (ITF) compliance certificate valid till 31st December. 2020:
e. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December. 2020;
f. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2020 or valid Certificate issued by BPP;
g. Sworn Affidavit disclosing:
• That the company is not in receivership, insolvent or bankrupt.
• That the company does not have any Director who has ever been convicted in any court in Nigeria or any other country for criminal offence in relation to fraud or financial impropriety.
• Whether or not any officer of the relevant committee of the FSTC, Ohanso or the Bureau of Public Procurement (BPP) is a former or present Director. Shareholder or has any pecuniary interest in the bidder.
• That all documents submitted for the bidding are true and correct in all particulars
h. Audited Financial Statement of the company for the past three years (2017,2018 and 2019) duly signed and sealed by relevant firm and the turnover as reflected in financial statements must correspond with that on the Tax clearance certificate;
i. Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria, indicating declaration Bank willingness to provide credit facility for the execution of the project or the contractor can fund the project if awarded;
j. Company Profile with the Curriculum vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications such as COREN, QSRBN, CORBON, ARCON etc.
k. Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards. Valuation Certificates, Job Completion Certificates and photographs of the projects;
l. List of Equipment with proof of Ownership/Lease (where applicable);
m. For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate. Pension Compliance Certificate, ITF Compliance Certificate. NSITF Compliance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner); and
n. All documents for submission must be transmitted with a covering/forwarding letter under the Company/Firms Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address and Telephone Number. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm

4. COLLECTION OF TENDER DOCUMENTS
The tender documents are to be collected from the Office of the Vice-Principal Special Duties. FGC, Ikirun between the working hours of 08:00 am and 04:00 pm daily upon the payment of a non-refundable tender fee of N10,000.00 per Lot to Federal Science And Technical College, Ohanso, Abia State State through REMITA Platform in any Commercial Bank, and present the teller for the College receipt.

5. SUBMISSION OF TENDER DOCUMENT
Prospective bidders are to submit bid for each of the lot desired, two Hard copies (Marked ‘Original & Copy”) each for both Technical & Financial Bids with soft copy of Financial bid only in MS excel format, packaged in two separated wax sealed envelopes clearly marked at the top right corner of each envelope stating the title of the project i.e. ’TECHNICAL BIDS FOR project Title and LOT No.’ & “FINANCIAL BIDS FOR Project Title and LOT No.” and at the reverse side the name of the company should be boldly written. The Federal Government of Nigeria Has received credit from the International Development Association and grant from the European Union Towards the cost of State Employment & Expenditure for Results (SEEFOR) Project. SECTION A: BACKGROUND
The Integrated Health Program (HP) – Nigeria is a USAID/Nigeria funded five-year flagship project which started in 2018 and is aimed at strengthening the health systems for improved access to and quality of PHC services in supported States in Nigeria.

IHP covers seven intervention areas: Health Systems strengthening; Maternal Health; New-born Health; Child Health; Reproductive Health/Family Planning: Nutrition, and Malaria.

The project will be implemented at the National level and in Ebonyi, Kebbi, Sokoto, FCT, and the Bauchi States.

1. This Invitation for Bids (IFB) follows the General Procurement Notice (GPN) for this project that appeared in UNDB online on June 12, 2012.

2. The Federal Government of Nigeria Has received credit from the International Development Association and grant from the European Union Towards the cost of State Employment & Expenditure for Results (SEEFOR) Project. Bayelsa State is participating in the project and intends to apply part of the proceeds of this credit and grant for payments under the agreement(s) resulting from this IFB: Procurement of Starter Packs for four hundred (400) Graduates of TVET Institutions. IFB Number: BY/SEEFOR/NCB/G/03/2020

3. The Bayelsa State SEEFOR Project, State Project Coordinating Unit (SPCU) serves as the implementing agency for the Project and now invites sealed bids from eligible Bidders for Procurement of Starter Packs for four hundred (400) Graduates of TVET Institutions.
