1.0 INTRODUCTION
The Nigeria Export Processing Zones Authority (NEPZA) in compliance with its statutory functions is desirous of executing its FY2020 capital projects. 3.0 ELIGIBILITY REQUIREMENTS
Interested Firms are required to submit Two (2) bound documents duly signed, paginated, separated by dividers and arranged in the order outlined hereunder:
(a) Evidence of Certificate of incorporation with the Corporate Affairs Commission (CAC) including Form CAC 1.1 or CAC2 and CAC7;
(b) Evidence of Company’s Income Tax Clearance Certificate for the last three (3) years valid till 31 December, 2020 with a minimum average turnover of N100 million for Category A.
(c) Evidence of current Pension Compliance Certificate valid till 31 December, 2020;
(d) Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31December, 2020;
(e) Evidence of Current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31 December, 2020;
(f) Evidence of registration on the National Database of Federal Contractors, Consultants and Service providers by submission of Interim Registration Report (IRR) expiring on 31/12/2020 or valid Certificate issued by BPP;
(g) Sworn Affidavit disclosing whether or not any officer of the relevant committees of the Nigeria Export Processing Zones Authority or the Bureau of Public Procurement is a former or present Director, Shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
(h) Company’s Audited Accounts for the last three (3) years (2017, 2018,2019);
(i) Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide facilities for the execution of the project when needed;
(j) Company’s profile with the Curriculum Vitae of key staff to be deployed for the project, including copies of their Academic/Professional qualifications;
(k) Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects;
(l) For Training/Empowerment programs. Evidence of Registration with Centre for Management Development;
(m) Lot 2: Evidence of current registration/accreditation with Computer Professionals Registration Council of Nigeria and National Information Technology Development Agency (NITDA);
(n) For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate IRR & Sworn Affidavit are compulsory for each JV partner);
(o) All documents for submission must be transmitted with a Covering/ forwarding letter under the Company’s/Firm’s Letter Head paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No) and e-mail address. For over nine years, the Lake Chad Basin (LCB) crisis – impacting Northeast (NE) Nigeria, the Lake Region of Chad, the Region of Diffa in Niger, and the far North Region of Cameroon-has exacerbated existing vulnerabilities, torn apart communities and families, constrained livelihood opportunities, destroyed infrastructure, and dismantled local government’s ability to respond to the needs of its citizens. To address the myriad of inter-related and chronic factors inhibiting the recovery of resilient, rebuilt communities in the L.C.B., CRS was awarded the two-year Stabilization and Reconciliation in the Lake Chad Region (STaR) Project, which is financed by the German Ministry for Economic Cooperation and Development (BMZ) through KW(German Financial Cooperation)

1. OVERVIEW
USAID’s Integrated Health Program(IHP) would like to invite qualified local entities, Nigerian Non-governmental Organizations (For-Profit and Not-for-Profit), non-governmental professional associations, training institutions and other qualified organizations to submit Expressions of Interest (EOI) with respect to technical, managerial and other aspects of the program described below. The EOI may come as a single entity application or as a consortium IHP is seeking interested eligible organizations that have interest in training in Ebonyi and or FCT to implement a grant for competency-based skills training of healthcare providers at Primary Healthcare Centres (PHCs) and private health facilities (PHFs) on child health, immunization and nutrition services in Ebonyi and or FCT. This training will use a Low Dose High frequency (LDHF) modular training approach and will involve onsite facility-based training, supervision and clinical mentoring of the health workers on child health thematic areas including Integrated Management of Childhood Illness (IMCI), Infant and Young Child Feeding (IYCF) and other areas of Nutrition and Routine Immunization. The roll out will be in IHP supported facilities in FCT and Ebony state as follows:

(i) Ebonyi State: 198 health facilities: 171 PHCs (the designated 1 PHC per Ward) and 27 Private Health Facilities (PHFs).
(ii) FCT: 124 health facilities: 62 PHCs (the designated 1 PHC per ward), and 62Private Health Facilities (PHFs)
This EOI is open to registered non-governmental organizations or a consortium of organizations in Nigeria who may or may not be resident in Ebonyi State or FCT but who are technically capable of providing the required services and willing and able to provide on-site training at the Primary Health Care facilities, General Hospitals and Private facilities that will be listed by IHP in Ebonyi state and the FCT.

The goal of IHP is to contribute to state-level reductions in child and maternal morbidity and mortality and to increase the capacity of health systems (public and private) to sustainably support quality primary health care services. IHP is providing technical assistance and capacity building to the State Ministries of Health in Bauchi, Kebbi, Sokoto, Ebonyi and FCT, State Primary Health Care Development Agencies (SPHCDA/PHCDB), Local Government Authorities/Area Councils, and Wards to apply state-of-the-art evidence, best practices and learning from within Nigeria and globally to deliver a fit-for-purpose capacity building approach to optimize health worker skills and confidence to deliver high-impact integrated RMNCH+NM services in the States.

IHP collaborates with other USAID implementing partners, such as Breakthrough Action- Nigeria (BA-N), Global Health Supply Chain – Procurement and Supply Management (GHSC-PSM) and the U.S. President’s Malaria Initiative for States (PMI-S) projects to increase demand for services and improve access to essential medicines and commodities, and to improve malaria case management.

(h) Sworn Affidavit disclosing whether or not any Officer of the relevant committees of the Nigeria Institute of Animal Science or the Bureau of Public Procurement is a formal or present Director, Shareholder or has any pecuniary interest in the Bidder or to confirm that all information presented in its bid are true and correct in all particulars;
(i) Company’s Audited Accounts for the last three (3) years – 2017, 2018, 2019;
(j) Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria indicating willingness to provide credit facility for the execution of the project when needed;
(k) For (Consultancy Services) Company resume, including two (2) key Technical Staff with required verifiable and professional qualification to be deployed for the execution of Contract;
(l) Company resume, including profile of at least Three (3) key Technical staff with the required skills/experience to be deployed for the execution of projects A(4-7) They must be Registered Animal Scientists, (RAS), with current Annual Practicing License (APL)
(m) Lot A1 & 3: Evidence of registration/accreditation by Computer Professionals Registration Council of Nigeria and National Information Technology Development Agency (NITDA);
(n) Evidence of Practicing License issued by ICAN, ANAN or other related professional body (LOT A3).
(o) For supply of equipment Letter of Authorization as representatives of the Original Equipment Manufacturers (OEM);
(P) Category A Evidence of Firm’s registration with relevant professional body(ies).
(q) Verification documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Job Completion Certificates and Photographs of the projects;
(r) All documents for submission must be transmitted with Covering/Forwarding Letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC). 1.0 INTRODUCTION
The National Theatre, Iganmu, Lagos, in an attempt to continue to restore the edifice to its past glory, is desirous of executing its Year 2020 Projects. In compliance with the Public Procurement Act 2007, It hereby invites interested experienced and registered contractors/consultants to submit bids and expression of interest for the following approved projects:

2.0 SCOPE OF WORKS/GOODS/SERVICES

3.0 ELIGIBILITY REQUIREMENTS
(a) Evidence of Certificate of Incorporation with Corporate Affairs Commission (CAC) including forms CAC2 and CAC7 (or CAC 1.1). Business Name is acceptable for Lot 3;
(b) Evidence of Company’s Income Tax Clearance Certificate (or Personal Income Tax Clearance Certificate of all Partners in case of Business Name) for the last 3 years (2017,2018 & 2019)expiring on 31 December, 2020;
(c) Evidence of Current Pension Compliance Certificate valid till 31 Dec. 2020.
(d) Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till Dec. 2020.
(e) Evidence of Current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31 Dec. 2020.
(f) Evidence of Registration on the National Database of Federal Contractors, Consultant and Service providers by submission of Interim Registration Report (IRR) expiring on 31/12/2020 or valid certificate issued by BPP.
(g) Sworn Affidavit:
(i) Disclosing whether or not any officer of the relevant committee of National Theatre or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars.
(ii) That the company is not in receivership, insolvent or bankrupt
(iii) That the company does not have Director who has been convicted in any court in Nigeria or any other country for criminal offences in relations to fraud or financial impropriety.
(h) Company’s Audited Accounts for the last three (3) years (2017,2018 & 2019).
(i) Evidence of Financial Capability to execute the project by submission of Reference Letter from reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed.
(j) Company’s Profile with the Curriculum Vitae or Key Staff to be deployed for the project including copies of their Academic/Professional qualification (in case of construction: COREN, DSRBN, ARCON, CORBON etc).
(k) Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects.
(l) List of Plants/Equipment with proof of ownership/Lease (where applicable).
(m) For Consultancy Service: evidence of Firm’s registration with relevant professional body(ies);
(n) All documents for submission must be transmitted with a covering/forwarding letter under the company/firms letter head paper bearing amongst others, the registration number (RC) as issued by the Corporate Affairs Commission (CAC), contact address, telephone number (preferably GSM no.), and e-mail address. The letter head paper must bear the names and nationalities of the Directors of the company at the bottom of the page, duly signed by the authorized officer of the firm.
