1. INTRODUCTION
The Ministry of Niger Delta Affairs (MNDA), in furtherance of its statutory mandate, intends to implement various Goods, Works and Services in the 2020 Capital Projects, in line with the proposed budget for the financial year 2020 and in accordance with the Public Procurement Act, 2007.

The Ministry, therefore, invites interested Contractors, Suppliers, Consultants, and Service Providers with requisite expertise and experience to submit Pre-qualification/Expression of Interest Documents and/or Technical/ Financial proposals, where applicable, for the projects of choice for the Procurement of works, goods, and services to be provided in the respective states of the Niger Delta Region.

Scope of activities under the External Storage Contractor Facilities include:

• Insertion of new tapes into the database
• Tape transcription Services
• Tape/Data Recovery services
• Scanning services

g. Sworn Affidavit disclosing whether or not any officer of the relevant committees of University of Calabar Teaching Hospital, Calabar or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
h. Company’s Audited Accounts for the last three (3) years- (2017, 2018 & 2019),
i. Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
j. Company’s Profile with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications (in case of construction: COREN, QSRBN, ARCON, CORBON etc.);
k. Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects;
l. Letter of Authorization from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria, for bidders of Lot 1:
m. Lot 2: List of Plants/Equipment with proof of Ownership/Lease (where applicable);
n. Lot 3: Letter of Authorization as representative of the Original Equipment Manufacturers (OEMs):
o. All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Nigeria Extractive Industries Transparency Initiative (NEITI) is mandated by law to promote transparency and accountability in the management of Nigeria’s oil, gas and mining revenues, NEITI is the national version of the Extractive Industries Transparency Initiative (EITI), which is a global movement aimed at ensuring that extractive resources aid sustainable development.

Pursuant to the above, NEITI intends to apply part of the funds approved under the 2020 Appropriation for the procurement of Consultants/Auditors for the continuation of its annual audits covering Financial, Physical and Process aspects of the Oil & Gas and Solid Minerals Industry.

LOT 1. OIL AND GAS INDUSTRY AUDIT 2020

SCOPE OF SERVICES
The scope of the services includes the following:
(a) Report payments by companies and receipts by relevant government agencies duly reconciled in accordance with the NEITI requirements.
(b) Report on the quantities of hydrocarbons (oil & gas and refined products, including condensates where appropriate) produced, exported and imported.
(c) Validate all payments by companies and government agencies to determine accuracy and completeness and report all underpayments and balances payable to the Federation.
(d) Review the policies and procedures of collection, custody and management of funds and make recommendations that will enhance the efficiency and transparency of the sector.
(e) Reporting will be under the New EITI Standard of 2019.

1. INTRODUCTION
The Federal Government of Nigeria wishes to invite interested Insurance companies to Bid for Group Life Assurance cover for Public Servants in compliance with the Pension Reform Act 2014 for the 2021/2022. The OHCSF hereby invites experienced and competent companies, to submit Expression of Interest in compliance with the Provisions of the Public Procurement Act 2007 for the implementation of the above as follows:

2. ELIGIBILITY REQUIREMENTS
Interested Companies are to submit the following documents:
i. Evidence of Certificate of Incorporation with the Corporate Affairs Commission (CAC) including Forms CAC2 and CAC7
ii. Evidence of Company’s Tax Clearance Certificate for the last three (3) years (Year (2017,2018 and 2019) valid till 31 December, 2020
iii. Evidence of License by the National Insurance Commission (NAICOM) valid till 31st December 2020,
iv. Evidence of claim settlements and outstanding claims in 2017, 2018 & 2019 on the Group Life Assurance cover of the OHCSF Scheme, a signed declaration by the Chief Executive Officer that the company settles and continues to settle all genuine claims promptly.
v. Evidence that the company has assets as stipulated in the Insurance Act 2003 and its (2018) published accounts for its life assurance business
vi. A certified true copy of its license or certification as an Insurer to transact life assurance business by NAICOM
vii. Evidence of current Pension Compliance Certificate valid till 31st December, 2020,
viii. Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2020;
ix. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate validity 31st December, 2020,
x. Evidence of Registration on the National Database of Federal Contractors. Consultants and Service providers by submission of Interim Registration Report (IRR) expiring on 01/01/2020 or valid Certificate issued by Bureau of Public Procurement (BPP)
xi. Copies of the company’s published annual report and account for the last 3 years certified by NAICOM (2017, 2018 and 2019)
xii. Sworn Affidavit disclosing whether or not any Officers of the relevant Committees of the OHCSF or the Bureau of Public Procurement is a former or present Director, Shareholder or as any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars
xiii. Company’s Audited Accounts for the least three (3) years (2017, 2018 and 2019).
xiv. Evidence of financial capability by submission of reference letter from a reputable commercial bank in Nigeria their qualification.
xv. Company’s Portfolio with the Curriculum Vitae of 6 Top Management Staff with and Insurance working experience.
xvi. Must have a verifiable address in Abuja, FCT.
xvii. Current Reinsurance program and premium payment for the class of insurance. xvi.
xviii. Verifiable Documentary evidence of Group Life experience in at least three (3) different scheme executed in the last five (5) years; xix. Value addition to the policy will be an added advantage; and
xix. All documents for submission must be transmitted with a Covering/Forwarding Letter Company/Firm’s Letter Head Paper bearing among others, the Registration Number (RC) issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail Address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company al the bottom of the page, duly signed authorized officer of the firm.

3. SUBMISSION OF EXPRESSION OF INTEREST TENDER DOCUMENTS:
a. Interested companies are to submit one (1) bound Expression of Interest (EoI) document separated by dividers and arranged as indicated above. The documents should be submitted in a sealed envelope at the address below and clearly marked with the name “INSURANCE BID” An electronic copy of the submission is to be made through email to the address below. Furthermore, the reverse of each sealed envelope should have the Name and Address of the bidder and dropped in the designated Tender Box situate at Room 6023, 6 floor, Block B. Federal Secretariat Complex, Phase ii not later than 12 noon on Monday, 19 October, 2020.
b. All bidders must fill the bid submission Register while submitting their Expression of Interest in the Tender Box situate at the designated location as the OHCSF will not be held liable for misplaced or wrongly submitted bids. i. Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project:
j. Company’s Profile with the Curriculum vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications such as COREN, QSRBN, ARCON CORBON, etc;
k. Verifiable documentary evidence of at least three(3) similar jobs executed in the last five (5) years including Letters of Awards and Job Completion Certificates;
l. List of Equipment with proof of Ownership/Lease Agreement;
m. For Joint Venture/Partnership. Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner), and
n. All documents for submission must be transmitted with a covering forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC). The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.
