INVITATION TO TENDER, PRE-QUALIFICATION AND EXPRESSION OF INTEREST
1. INTRODUCTION
The Federal Medical Centre, Abeokuta, established to provide medical services to the gateway state of Ogun and other neighbouring states and nations. The hospital prides itself on excellence in research, training and health care service delivery. The FGN has allocated public funds for its 2024 capital projects. Experienced and reputable contractors are hereby invited to submit bids for the following procurements.

3. ELIGIBILITY REQUIREMENTS
Suitable qualified companies are invited to submit three (3) bound copies of prequalification documents duly signed, paginated, separated by dividers and arranged in the outlined hereunder;
a) Evidence of Certificate of Incorporation issued by Corporate Affairs Commission (CAC) including Forms CAC 1.1 or CAC 2 and CAC7. Business name with form BN1 is also acceptable of service;
b) Evidence of Company’s Income Tax Clearance Certificates (or personal Income Tax Clearance Certificates for all partners) for the last three (3) years valid till 31st December 2024; with a minimum annual turnover of N200M for Radio Diagnostics Centre and N100M for others, as stated in the TCC & concurrent with the audited accounts).
c) Evidence of Pension Clearance Certificate valid till 31st December, 2024 (this requirement is only applicable to bidders whose number of staff is 15 and above).
d) Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2024 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is N50m and above);
e) Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2024;
f) Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2024 or valid Certificate issued by BPP;
g) Sworn affidavit
• disclosing whether or not any officer of the relevant committees of the Federal Medical Centre, Abeokuta or Bureau of Public Procurement (BPP) is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
• That no director has been convicted in any county for any criminal offence relating to fraud impropriety or criminal misrepresentation of falsification of facts relating to any matter.
• That the company is not in receivership, the subject of any form or bankruptcy proceedings of the subject of any form of winding up petition or proceeding
h) Company’s Audited Accounts for the last three years (2021 , 2022 & 2023)
i) Reference letter from a reputable commercial bank indicating willingness to provide credit facility for the execution of the project when needed;
j) Company’s profile with the curriculum Vitae of key staff to be deployed for the project, including copies of their academic/professional qualifications (in case of construction/Rehabilitation works: COREN, QSDRBN, ARCON, CORBON etc);
k) Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including letters of awards, valuation certificates, job completion certificates and photographs of the projects;
l) Health & Medical Equipment: Letter of Authorization from the Original Equipment Manufacturers listed in the approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria;
m) Evidence of verifiable OEMs Service Centre in Nigeria to ensure after sales maintenance in line with the approved SGF circulars enforcing aftersales maintenance services for Medical equipment, duly certified by NNRA & SONCAP for radiology equipment;
n) Works: List of Plants/Equipment with proof of Ownership/Lease agreement.
o) Consultancy services: evidence of firm’s current registration issued by the relevant regulatory professional body(ies) such as ICAN/ANAN, CPN & NITDA;
p) Lot M: Evidence of firm’s current registration with the office of the Auditor-General for the federation.
q) For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificates, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR & Sworn affidavit are compulsory for each JV Partner);
Note: All documents for submission must be submitted with a Covering/Forwarding letter under the Company’s letter head paper bearing amongst others, the Registration Number (RC) as issued by the CAC, Contact Address, Telephone Number (preferably GSM No) fax and email address. The letter head paper must bear the Names and Nationalities of the Directors of the company at the bottom of the page, duly signed by the authorized officer of the firm
Note that all documents/information required must be paginated, submitted in the sequence/order as listed above.
4. SUBMISSION OF PRE-QUALIFICATION DOCUMENTS
Interested firms are to submit three (3) bound copies of Pre-qualification-documents, separated by dividers and arranged as indicated above. The envelope should be addressed to The Medical Director, Federal Medical Centre, Idi-Aba, Abeokuta and clearly mane (the name of the project and the Lot number).
Furthermore, the reverse of each sealed envelope should bear the name and address of the bidder and should be dropped in the designated Tender Box located at the Office of the Medical Director, Federal Medical Centre, Idi- Aba, Abeokuta not later than 10:00 a.m. Monday, 13 May 2024.
Please ensure that you sign the Bid Submission Register as the Hospital will not be held liable for misplaced or wrongly submitted bids. For further enquiries, please contact the Head of Procurement.
5. OPENING OF PRE-QUALIFICATION DOCUMENTS
The Pre-qualification documents will be opened immediately after the deadline for the submission by 10:00a.m. Monday, 13 May, 2024 at Telemedicine Hall, Federal Medical Centre, Abeokuta, Olabisi Onabanjo Way, Idi-Aba, Abeokuta, Ogun State.
6. COLLECTION OF TENDER DOCUMENTS (INVITATION TO TENDER)
Tender documents are to be collected from the Head of Procurement’s Office, 1″ floor, Finance, Procurement and Audit Complex, Federal Medical Centre, Idi-Aba, Abeokuta between the hours of 8.00am and 4.00pm (Monday-Friday) upon presentation of authenticated payment receipt of a non-refundable tender fee of N10, 000.00 (Ten Thousand Naira) only per Lot, paid into the Federal Medical Centre, Idi-Aba, Abeokuta’s remita account in any reputable Commercial Bank in Nigeria.
7. SUBMISSION OF TENDER DOCUMENTS (INVITATION TO TENDER)
Two hard copies and softcopy in a flash drive of Technical & Financial bids should be packaged in two (2) separate envelopes and clearly marked “Technical Bid” or “Financial Bid” (with the name of the project and the Lot number).
The two (2) envelopes should be put into a bigger envelope addressed to The Medical Director, Federal Medical Centre, Idi-Aba, Abeokuta and clearly marked “Technical and Financial Bid” with name of the project and the Lot number”.
Furthermore, the reverse of each sealed envelope should bear the name and address of the bidder and should be dropped in the designated Tender Box located at the Office of the Medical Director, Federal Medical Centre, Idi- Aba, Abeokuta not later than 10:00a.m. Tuesday, 28 May, 2024.

8. OPENING OF TENDER DOCUMENTS (INVITATION TO TENDER)
The technical documents will be opened immediately after the deadline for the submission by 10:00 am. Tuesday, 28 May, 2824 at Telemedicine Hall Federal Medical Centre, Olabisi Onabanjo Way, Idi-Aba Abeokuta, Ogun State.
9. GENERAL INFORMATION
a. All EOls/tender documents must be in English Language and signed by an official authorized by the bidder;
b. All EOIs/tender documents submitted after the deadline for submission would be returned un-opened,
c. All costs will be borne by the bidders;
d. Applicants should not hid for more than two (2) lots, violation is disqualification in all lots;
e. The EOls Prequalification documents and technical bids will be opened in the presence of Bidders or their representatives, CSOs in the areas of Anti-Corruption and Transparency and Private Professional bodies, and will be covered by video recording, invitation link will be sent to bidder who chooses to join the EOls/bid opening online:
f. Invitation for Prequalification: Only pre-qualified bidders at technical evaluation will be invited at a later date for collection of tender documents,
g. Invitation to tender: Only prequalified bidders at technical evaluation will be invited at a later date for financial bids opening, while the financial bids of un-successful bidders will be returned un-opened
h. Consultancy services: Only Shortlisted consultants will be invited at a later date for collection of Request for proposals;
i. The Federal Medical Centre, Abeokuta is not bound to prequalify and bidder reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with section 28 of the Public Procurement ACT 2007.
