LSMOH-PROCUREMENT OF ANTI-MALARIA DRUGS

1. BACKGROUND
The Lagos State Ministry of Health has secured necessary approval from Mr. Governor in order to achieve its State Malaria Elimination Programme (SMEP) and promote a Malaria Free Lagos.

2. This National Competitive Bidding (NCB) process as specified by the State Procurement Guidelines is open to all bidders as defined in the Public Procurement Guidelines – www.lagosppa.gov.ng.

3. SCOPE OF WORK
The Lagos State Ministry of Health now invites sealed bids from eligible contractors for the contract below as detailed in the bidding document:

4. GENERAL QUALIFICATION REQUIREMENTS FROM INTERESTED PARTIES
i. Evidence of Registration with Corporate Affairs Commission (CAC) with inclusion of Certificate of Incorporation and Articles of Association.
ii. Company profile and Organizational structure with full name of organization and contact person, postal address, telephone/fax numbers, and e-mail addresses.
iii. Evidence of company tax clearance for the last 3 years (2017, 2018 &2019)
iv. Audited financial statements of the company for the years (2017, 2018 &2019)
v. Evidence of 3 years development levy for two directors (where applicable)
vi. Copies of VAT Registration with TIN number and evidences of VAT remittances for the last 3years. vii. Evidence of available financing and/or access to credit line.
vii. Verifiable evidence of 2 similar experiences (within a minimum of 3 years) with government, private or non-governmental organisations. These should include evidence of satisfactory completion or progress for each experience indicated).
viii. A sworn Affidavit that all documents submitted is genuine and verifiable.
ix. Evidence of registration with Lagos State Government (PPA) in the appropriate class
